Integrating Psychological and Behavioral Care into Periodontal Practice: A Narrative Review and Proposed Clinical Care Protocol

Periodontitis is a chronic inflammatory disease in which sustained plaque control, risk-factor modification, adherence, and supportive care are central to long-term stability. Psychological stress, general anxiety, dental anxiety, and low self-efficacy may adversely affect health behavior and may also modulate periodontal treatment response. However, psychological and behavioral support is not consistently operationalized within periodontal pathways. To synthesize clinically relevant evidence on psychological and behavioral determinants of periodontal care and to propose a pragmatic interdisciplinary protocol integrating brief screening, behavioral support, stress/anxiety management, and referral into conventional periodontal treatment. A focused narrative review was undertaken using PubMed/MEDLINE and citation chaining through August 2026. Priority was given to periodontal clinical practice guidelines, systematic reviews/meta-analyses, and longitudinal or interventional studies relevant to psychological stress, general anxiety, dental anxiety, self-efficacy, adherence, motivational approaches, cognitive-behavioral principles, and oral-hygiene behavior. Evidence was translated into seven clinical components using three prespecified conceptual principles: consistency with evidence-based periodontal care, proportionality to identified psychosocial need, and feasibility in routine dental settings. This was not a systematic review and no PRISMA process, formal risk-of-bias assessment, or meta-analysis was undertaken. Evidence supports repeated behavior-change support and indicates associations between psychological stress/anxiety and periodontal disease or treatment response, although certainty and intervention effects remain heterogeneous. Dental anxiety is clinically distinct because it may directly affect avoidance, attendance, procedural tolerance, and adherence. The proposed pathway uses an authors’ stepped-care framework: routine behavioral support for all patients, enhanced support for persistent behavioral barriers, and mental-health assessment/referral when clinically significant psychological needs are identified. Structured psychological and behavioral care is a plausible adjunct to contemporary patient-centered periodontal management. The proposed protocol is an evidence-informed, testable clinical framework; its feasibility, acceptability, and effectiveness require prospective validation.
