Localización dental y detección de anomalías en radiografías panorámicas basadas en aprendizaje profundo

El análisis automatizado de radiografías panorámicas sigue siendo un desafío debido a la complejidad anatómica y la variabilidad de las imágenes. Aunque el aprendizaje profundo ha demostrado un alto rendimiento en el análisis de imágenes dentales, la mayoría de los estudios se centran en tareas aisladas. El objetivo fue proponer un marco jerárquico basado en YOLOv8 para el análisis integral de radiografías panorámicas mediante anotaciones dentales estructuradas. Se desarrolló una canalización de aprendizaje profundo de tres etapas basada en YOLOv8 utilizando el conjunto de datos DENTEX. El marco comprende: (1) la clasificación por cuadrantes, (2) la enumeración dental según el sistema FDI (11-48) y (3) la detección de anomalías a nivel dental mediante un enfoque de dos etapas (cribado binario seguido de la clasificación por subtipos). Se utilizaron radiografías panorámicas con anotaciones jerárquicas, divididas en conjuntos de entrenamiento y validación en una proporción de 80:20. El rendimiento se evaluó mediante mAP50, mAP50-95, precisión (precision), sensibilidad (recall) y puntuación F1. El modelo alcanzó un rendimiento casi perfecto en la clasificación por cuadrantes (mAP50=0.994, mAP50-95=0.750, precisión=0.994, sensibilidad=0.995 y F1=0.994), un rendimiento elevado en la enumeración dental (mAP50=0.936, mAP50-95=0.536, precisión=0.902, sensibilidad=0.897 y F1=0.899). La detección de anomalías mostró un rendimiento moderado (mAP50=0.687, mAP50-95=0.480, precisión=0.655, sensibilidad=0.742 y F1=0.696). A nivel de clase, los dientes impactados (F1=0.904) y la caries dental (F1=0.885) fueron detectados con alta precisión, mientras que las lesiones periapicales (F1=0.568) y la caries profunda (F1=0.585) presentaron un rendimiento inferior. La precisión y la sensibilidad se mantuvieron equilibradas en todas las tareas. El marco jerárquico propuesto permite la localización anatómica y la integración de múltiples tareas de detección en radiografías panorámicas dentro de una canalización unificada basada en YOLOv8. Aunque el rendimiento fue casi óptimo para las tareas anatómicas, la detección de enfermedades continúa siendo un desafío, especialmente en lesiones de bajo contraste.
