Abstract

Changes in the primary care models had been pointing the importance of moving from a paternalistic model to independent model that promote the autonomy of the patients where their right to information access is reinforced as a strategy to make concrete and informed decisions. The act of delivering bas news must take place from and individualized approach, with adequate and assertive communication strategies and following a series of basic guidelines that guarantee effectiveness. The way we deliver bad news will have an impact in the illness perception of the patient and their support network, as well as on their ability to cope with their condition. Communication protocols developed to deliver bad news can be a useful tool that allows appropriate interventions, thus strengthening the doctor-patient relationship, as well as helping to reduce anxiety on both sides. These strategies can be carried out in a few minutes and require basic training which can be perfected over time and experience.

Keywords: bad news; clinical communication; doctor-patient relationship, SPIKES protocol.
